  2. Fully accomplished Personal Data Sheet (PDS) 
  3. Certified True Copy/photocopy of Transcript of Records (highest educational attainment)
  4. Certified True Copy/photocopy of Diploma (highest educational attainment)
  5. Certified True Copy/photocopy of Eligibility (if applicable)
  6. Certificate of Trainings and/or Seminar (recent past 5 years)
  7. Certificate of Employment
  8. Work Experience Sheet 
  9. Certificate of Performance (for REPS)
  10. IPCR for the last rating period (for ADMIN) (if applicable)
