Processing of Original Appointments
Requirements
The following documents should be attached to the recommendation for original appointments:
- Fully accomplished basic papers (HRDO B-1)
- Personal Data Sheet with two 2 x 2 pictures (guide)
- Justification
- Original Transcript of Records (TOR) and Diploma or True Copy of Grades certified by the College Secretary, if TOR and Diploma are not available
- NBI clearance
- Marriage Certificate, authenticated by NSO, if married ∙ Birth Certificate, authenticated by NSO
- Medical Certificate with documentary stamp
Additional documents are required in the following cases:
- For Teaching Associates/Teaching Fellows: Photocopy of Form 5
- For those who transfer from another government office/institution: Complete statement of Service Record, Certification of leave credit, Certification of last salary received, approved transfer and clearance from former agency
- For foreigners: Working visa and employment permit from Department of Labor and Employment (DOLE), if there is no MOA with reciprocal terms
Payment of first salary
The following documents should be submitted to HRDO to facilitate payment of the faculty member’s first salary:
- Approved Appointment (latest NOSA/Renewal if applicable)
- Oath of Office
- SALN
- COS/DTR/approved Leave if Any
- Report for Duty (Faculty/REPS) / Assumption to Duty (Admin)
- DBP Account Nakpil Branch (Photocopy of ATM Card)
- Pag-ibig MID No. (UPM as Employer) - MDF
- PhilHealth NO. (UPM as Employer) - MDR
- GSIS Membership information sheet only ( GSIS ARA c/o HRDO)-BP#/ARA if no BP# (note: if Transfer from other Agency and with BP#, pls attach ARA also)
- BIR Form:
- 1 -1905 verified by BIR if with TIN
- 2 - 1902 with Birth Certicate and Marriage contract if Applicable and submitted directly to Accounting for filing of TIN (1902 should not be submitted with the requirements for first salary, it should be submitted directly to Accounting Office) Birth Certificate/Marriage Contract - issued by PSA.

Additional Requirements for New/transfer employees from Other Government Agency:
- Certificate of Last Salary and Other benefits Received
- Approved Clearance

NOTE:
- ALL DOCUMENTS MUST BE CERTIFIED TRUE COPY if it's NOT ORIGINAL COPY
- PLEASE SUBMIT TO ACCOUNTING ON OR BEFORE THE 15TH OF THE MONTH FOR INCLUSION IN THE SPECIAL PAYROLL AND GENERAL PAYROLL
