List of Requirements before releasing the new ATM Cards: 1. Duly filled up Bank’s Customer Record Forms*; 2. Two (2) pcs. Latest 1 X 1 ID Pictures; 3. Photocopy of two (2) current and unexpired Valid ID’s (UPM issued ID and one (1) Primary ID); 4. Copy of Latest Employment Contract / Plantilla / Appointment Record with UP Manila *Bank’s Customer Record Forms are available on the following areas: - UP CASH / CAD - UP NIH - DBP Manila-Nakpil Branch Note: a. Kindly ensure submission of complete documents to avoid delay in ATM Card release. Cards not claimed after December 31, 2023 shall be subject to perforation. b. You can still use your old ATM card until September 30, 2023. Hence, automatic activation of the new ATM card will be on October 01, 2023. Your PIN on your New ATM card will be the same with your previous PIN you are using in your old ATM card. Please be guided accordingly.
